Registration Form Info:
This form needs to be completed FIRST and submitted to the office with the $60.00 Registration Fee. This fee is non-refundable.

Medical Form
This form needs to be completed, signed, and dated by your child's physician. The date on it must be current. After 1 year, this form expires and a new one needs to be completed and signed by the physician again.
Licensing Form
This form needs to be filled out completely. No blank spaces, please! This form can be redundant, but it is necessary to complete each question.
Transportation Release Form
On this form you are to list all persons that can pick up your child from school. You can make modifications to this form through out the school year, if necessary. If someone comes to pick up your child that is not on this list, your child WILL NOT be released. Additionally, if the person coming to pick up your child is on the list and they are picking up for the first time, they MUST have a picture I.D. with them to prove their identification. If they do not have proper identification, your child WILL NOT be released.
